What to Bring

During your first visit, we’ll gather important background information — like your medical history — and give you time to get to know your provider. Please arrive a few minutes early so we can complete your initial paperwork, and bring:

  • A list of current prescriptions and over-the-counter medications, including dose and frequency
  • Information about your medical and surgical history
  • Any recent test results, X-rays, or relevant records
  • Your insurance information — we offer complimentary verification

We’re open Monday through Thursday, 9:00am–6:00pm, and closed Friday through Sunday. You can request an appointment any time using the form above and we’ll get back to you.
